Team-Mapping

Our team applied team-mapping methods that has been developed and previously applied in clinics across British Columbia by two of our researchers from UBC’s Innovation and Support Unit.  The team-mapping session involved patient partners and care team members in knowledge sharing activities.

Circle of Care Mapping Example

The circle of care mapping image is an example for Barbara.  It is meant to demo the approach in mapping out a team for a patient, and how it may look in a Team Mapping facilitator workshop. This circle of care map may look differently depending on who is at the table mapping the team. For our study, we will had patient partners, family physicians, a nurse, social worker and medical office assistants at the virtual session.
