This project explores the feasibility of co-developing a Learning Health Community (LHC) in the Comox Valley region (Comox Valley, Courtney, Cumberland) to support people who care for older adults living with dementia. A Learning Health Community brings together caregivers, community members, and service providers to share knowledge, generate data, and co-create tools that improve health system navigation and access to support.
Grounded in local priorities, this project responds to increasing challenges faced by informal caregivers, especially in rural and aging communities. By strengthening community learning cycles and integrating digital health tools, the LHC aims to improve connections, reduce isolation, and inform broader health system improvements that reflect real community needs.
